Wegovy Tablets (semaglutide HD-oral): the 2026 GLP-1 guide

Overview
Wegovy Tablets is Novo Nordisk’s once-daily 25 mg oral semaglutide tablet, FDA-approved on December 22, 2025 for chronic weight management — the first oral GLP-1 specifically approved for weight loss (separate from Rybelsus, which is the lower-dose oral semaglutide for type 2 diabetes).
This is the same active ingredient as injectable Wegovy (semaglutide 2.4 mg subcutaneous), reformulated as an oral tablet using the SNAC absorption enhancer (salcaprozate sodium). At the higher 25 mg oral dose (≈10× the Rybelsus maintenance dose), Wegovy Tablets achieves systemic semaglutide exposure comparable to subcutaneous Wegovy.
It is one of the two oral GLP-1s available for weight management in 2026, alongside Eli Lilly’s Foundayo (orforglipron). Wegovy Tablets requires a 30-minute fasting window; Foundayo does not.
Clinical Efficacy
In OASIS-4 (307 adults with obesity or overweight + comorbidity, no T2D, 64 weeks), Wegovy Tablets 25 mg produced 13.6% mean body-weight loss (treatment-policy estimand) vs 2.2% on placebo. The on-treatment estimand was 16.6%1. The trial was the basis for FDA approval.
This places Wegovy Tablets at the higher end of the oral GLP-1 class — slightly above Foundayo (~12%) and meaningfully above lower-dose oral semaglutide options (Rybelsus ~4–8%). It remains below the injectable tirzepatide (Zepbound ~22%) and weekly subcutaneous semaglutide ([Wegovy injection] ~15%).
Dosing & Titration
Four approved tablet strengths:
- 1.5 mg — initiation, weeks 1–4
- 4 mg — escalation, weeks 5–8
- 9 mg — escalation, weeks 9–12
- 25 mg — maintenance dose
Schedule: increase the dose every 4 weeks until reaching 25 mg as tolerated. The titration mirrors the subcutaneous Wegovy schedule but uses tablet strengths matched to oral bioavailability2.
Critical: take on an empty stomach. Swallow whole with no more than 4 oz of plain water at least 30 minutes before the first food, beverage, or other oral medication of the day. Skipping the fasting window can reduce semaglutide absorption by 50% or more.
Side Effects
Boxed warning — Risk of Thyroid C-Cell Tumors
In rodents, semaglutide causes thyroid C-cell tumors at clinically relevant exposures; whether Wegovy Tablets causes the same in humans (including medullary thyroid carcinoma, MTC) is unknown. Wegovy Tablets is contraindicated in patients with a personal or family history of MTC or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

GI events were generally mild-to-moderate and transient, peaking during titration. Permanent treatment discontinuation due to adverse events was 6.9% on Wegovy Tablets vs 5.9% on placebo. Serious adverse events were lower with Wegovy Tablets (3.9%) than placebo (8.8%) — a notable finding likely reflecting the metabolic-disease burden in the placebo arm1.
The high nausea/vomiting rates (vs ~20%/9% on Foundayo and ~20%/8% on Rybelsus 14 mg) are consistent with the higher 25 mg dose and the daily peak/trough exposure of an oral formulation.
Contraindications
- Personal or family history of medullary thyroid carcinoma (MTC)
- Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
- Known serious hypersensitivity to semaglutide or excipients
Post-marketing reports for the broader semaglutide class note acute pancreatitis, acute kidney injury (often related to GI fluid loss from prolonged nausea/vomiting), and pulmonary aspiration risk during anesthesia.
2026 Availability
Launched broadly in January 2026 across NovoCare (manufacturer-direct), retail pharmacies (70,000+ locations), and major telehealth partners (Ro, Hims & Hers, PlushCare).
Cash from $149/month via NovoCare (manufacturer-direct self-pay, comparable to LillyDirect’s Foundayo program). Cash list price ~$1,049/month at retail without the NovoCare program.
Medicare GLP-1 Bridge: $50/month for the 25 mg tablet from July 1, 2026 onward, for Part D beneficiaries with BMI ≥ 27 + comorbidity (or ≥ 35 alone). The lower titration doses (1.5 / 4 / 9 mg) are not separately priced under the Bridge — patients receive them as part of the standard titration to the 25 mg maintenance dose.
For commercial-insured patients, Wegovy Tablets is on most major plan formularies starting Q1 2026, typically Tier 3 with prior authorization. Manufacturer savings card information was not yet published at launch.
